FLEP Application FormFLEP Program Application Form (595 K)
This is the form you will need to send in to your local DEC office to sign up for Practice Cost-Sharing under the FLEP program.
For further information on the FLEP Practices available and their associated Cost-Share rates, see the publication below where I have given them in a table.

This form is provided as a fillable form, PDF format, document.
FLEP Cost Share Rates TableFLEP Practices, Components and Cost Share Rates Table (56 K)

This Table provides the FLEP Practices, Components, and Component Codes available and their associated Cost-Share rates.

FLEP Daily Work LogFLEP Daily Work Log Form (23 K)
You need to file this form with your local DEC office, at the completion of a Practice, for a cost basis for work done under the FLEP program by yourself, family and/or friends.  Note that the cost-basis rates for the Chainsaw, Tractor, and Heavy Equipment groups, include the labor for the operator - Labor for those catagories is not added in the Labor column.
This form is provided as a fillable form, PDF format, document.
FLEP PracticesFLEP Practices (192 K)
This is a compilation of the 11 separate PDF documents on NYS DEC's site descibing the 11 FLEP Practice areas allowed under federal law.  I put them in one document for ease of reading, everything is in one place and each practice is bookmarked, and faster downloading - the total document size is less than the size of any one of the DEC documents describing one practice.

FLEP Practice StandardsFLEP Practice Standards (117 K)
This is a compilation of the 7 separate PDF documents on NYS DEC's site descibing the FLEP Practice Standards that were taken from NRCS Practice Standards.  These are referenced in the above "FLEP Practices" document and are a good reference and starting point for what should be done under FLEP Practice Standards.

I have linked three websites below to provide more information on the FLEP program so that I don't have to reiterate the details they contain.  I hope these are of help:
